• Photocopy of Valid ID (With 3 Specimen Signature)
• 4pcs 1×1 picture (formal attire w/white background)

• At least High School Graduate
• 18 – 50 years old

(ALS Graduate, Exited List na ng 4Ps, magulang ng profiled Child Laborer at profiled Malnourished children, mga dating OFW)
